Valtox provides mobile drug and alcohol testing, including DOT and non-DOT testing, on a scheduled basis. All services are subject to availability and confirmation.

2. Payments

• All payments are due at time of booking unless otherwise agreed upon.

• Online pricing includes applicable testing and reporting fees.

• Additional charges apply for confirmation lab testing, as disclosed.

3. Appointment Changes & Cancellations

• Appointments may be rescheduled with at least 24 hours’ notice.

• Cancellations within 24 hours may be subject to a cancellation fee.

• No-shows or late arrivals (15+ minutes) may forfeit the appointment fee.

4. Refunds

• Services rendered are non-refundable.

• Refunds may be considered for duplicate charges or system errors.

5. Test Results

• Non-negative results may require confirmation testing at an additional fee.

• Results are confidential and shared only with authorized parties.

• DOT and non-DOT testing complies with all federal and state requirements.

6. Limitation of Liability

Valtox is not liable for any damages arising from the use or inability to use our services, or from decisions made based on testing results.
